In-Home Services

When most people think of home health care, they most often imagine only physical injuries or illnesses. However, mental health is an important part of our overall health and can directly affect our physical well being or health. Mental illness requires diagnosis and treatment just as physical conditions do.

Health conditions are diagnosed and treated based on signs and symptoms, as well as on how much the condition affects a person’s daily life. Signs and symptoms can affect; behavior, feelings and thinking.

Support Coordination

Support coordinators help people with behavioral disabilities
and their families identify and obtain needed services.  They also advocate for, monitor, and evaluate services along with the individuals, their families or guardians.

A key role of the support coordinator is to assist people with the process and paperwork necessary to obtain services.  The system of multiple agencies providing services to persons with disabilities can be complex and confusing.

Support coordinators provide a “single point of entry” into services.
